Once it is
determined that we will work together, I will forward a packet
of information, a questionnaire and an agreement. This information
will aid me in determining what is important to you.
I ask you
to complete the material and return it to me. Upon receiving the
completed material, I will call to set up a mutually agreed upon
day and time for our first session.
